The University of Ibadan has announced a 24-hour mop-up exercise for returning students who missed the earlier submission of the University Health Service (UHS) Requisite Form.
According to a notice issued on March 6, 2026, this is specifically designed for students who failed to complete the initial health service form required by the university.
The mop-up form was made available on the same day and will remain open until 11:59 p.m. on Friday, March 6, 2026, giving affected students a final opportunity to comply with the requirement.In the statement signed by the Dean of Students, Demola Lewis, returning students were urged to complete the form promptly using the link provided in the notice.
The Student Affairs Division also outlined important instructions for students participating in the mop-up exercise. Students are required to upload a scanned copy of their Jaja Card while completing the form. The notice further warned that any falsification of information would attract severe penalties in accordance with the university’s regulations.
The UHS Requisite Form is part of the university’s health documentation process, aimed at ensuring that students’ medical records are properly updated within the institution’s health system. The mop-up exercise therefore serves as a final window for affected students to regularize their health records and prevent any complications.University authorities advised all concerned students to complete the process, emphasizing that no further extension may be granted after the closure of the mop-up exercise.
