The University of Ibadan has directed returning students who intend to apply for accommodation for the 2025/2026 academic session to complete the University Health Service (UHS) Requisite Form.
The directive was contained in a notice issued by the Student Affairs Division, which stated that completion of the health form is a compulsory requirement for consideration for hostel accommodation.
According to the notice, the UHS Requisite Form will close by 11:59 p.m. on Wednesday, February 11, 2026, with no extension. Students are also required to upload a scanned copy of their Jaja Card during the application process.
The university further noted that all final-year students and those in concessional categories, including MBBS students and physically challenged students, must complete the form before they can be considered for accommodation.
It warned that falsification of information will attract severe penalties in line with university regulations.
